Baylor Scott & White Memorial Hospital in Temple, TX, is seeking a Full Time Patient Navigator Registered Nurse to join our Ortho Trauma Progressive Care Unit (5 South). This is a 35-bed monitored nursing unit that provides the opportunity to work with a wide variety of patients ranging from adults to Geriatrics. The conditions that are within the scope of service include the following but are not limited to:
Post ICU trauma patients (serious traumatic injury to 2 or more body systems)
Complex Plastic reconstructive surgeries (including reconstructive flaps and graft sites, post cancer reconstruction, and trauma related reconstruction)
Complex ENT patients including free muscle flaps and neck dissections.
Complex ENT reconstructive tracheostomies and laryngectomies
Traumatic Complex Fractures and musculoskeletal wounds
Elective Joint Replacements - Rapid Recovery Program
Sports Injuries
Spinal Deformities
Complicated Pelvic, Hip, and compression Fractures
Skeletal Malformations and Abnormalities
Amputations and Prosthesis
Neurosurgery, Vascular Surgery, General Surgery
Medical/Surgical patients including those requiring Telemetry
Goals of the Patient Navigator Nurse:
Help to reduce the length of stay- advocating and eliminating barriers that would prevent a patient going home safely.
Throughput/ discharges- help to facilitate speedy discharges - utilizing discharge lounge/ HUB
Executive presence- will facilitate MDR and collaborate with interdisciplinary team members- will require peer to peer accountability
PNT will see all new admissions within 24-48 hours - introduce themselves to the patient, explain role, and determine if there are any barriers that would prevent the patient from returning to home or where they were staying previously
Improve patient satisfaction- nursing communication, patient education, and discharge readiness

Requirements of the Ideal Candidate
·Graduate of RN School of Nursing
·ADN required
·Active RN License
·Basic Life Support (BLS) Require or BLS within 30 days of hire or transfer
Qualifications
A Bachelor's degree.
At least 3 years of relevant experience to the table.
Registered Licensed Nurse.
You have, or are willing to obtain within your first 30 days, a Basic Life Support (BLS)
