As the Ortho Geriatric Nurse Practitioner, your responsibilities Include:
Identification of new Geriatric Fracture Program patients admitted to Cedars-Sinai.
Administration of the GFP master database.
Clinical oversight of GFP patients care and coordination with the multidisciplinary team:
a. Maintain fidelity of the Geriatric Fracture Program, ensuring principles set forth in GFP framework are being followed by the multidisciplinary team.
b. Coordinate, lead and document the daily GFP multi-disciplinary huddles.
c. Daily rounding and coordination of care with attending physicians.
d. Educate patients, care-givers and family members on trajectory of care during hospitalization and recovery, while incorporating patient and family goals of care into discussion.
e. Liaise with other inpatient services (supportive care, psychiatry, neurology, nephrology, cardiology, etc.) for patient care needs.
f. Provide ortho-geriatric inpatient clinical support to orthopaedic surgeons, floor nurses and CPs
g. Provide osteoporosis patient education to GFP and non-GFP patients with fragility fractures on 7 north.
h. Develop and provide education for nurses and CPs on geriatric topics of interest.
i. Develop and support nursing-led research initiatives.
j. Provide support for occasional osteoporosis and falls reduction community outreach.
