We have an exciting opportunity to join our team as an Adult Primary Care Nurse Practitioner - Bellevue.
In this role, the successful candidate As part of the continued Primary Care Medical Homes (PCMH) transformation in Adult Primary Care, the Nurse Practitioner (NP) will work within one of our continuity firms, providing access for new patients, as well as provide support to existing provider panels by offering same-day availability appointments to patients. Both of these functions would help improve clinic wait times for new and `urgent appointments. The NP would be scheduled to work on Saturdays. NP Schedule is Tuesday - Saturday
- Provide same-day access for patients with Bellevue PCPs in their Firm who are unavailable
- Review all ordered laboratory, radiographic and ancillary studies
- Document medical evaluation in QuadraMed Electronic Health Record (EHR)
- Communication/coordination of results with PCP and patients
- Assist with medication refills, patient forms and laboratory re-orders
- Participate in inter-disciplinary care management of high risk patients
- Evaluate new patients with focused history and physical examination
- Order appropriate laboratory, radiographic and ancillary studies, as indicated
- Make referrals to specialists, as indicated
- Document evaluations in QuadraMed EHR
- Participate in GIM quality improvement activities
- Review charts/reports for collaborative care and population health management
- Work one Saturday morning each week
To qualify you must have a Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New York
Education: Master's Degree in Nursing
Establishes a written Collaborative Practice Agreement with an NYUHC attending physician;Credentialed through the Credentialing Committee of the Medical Board with Delineation of Privilege approved with role specific treatment privileges
Certification required (as soon as feasible following graduation from NP graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice.
- ACLS, PALS, NRP Provider;(Neonatal Resuscitation Program), NALS per delineation of privileges; PRI Certification/Screening is optional;
Competence: At least five years of clinical nursing practice in the care of the patient population served; ability to apply nursing process toward achievement of specific outcomes in an organized fashion, which addresses patient/family safety and satisfaction. Previous experience as a nurse practitioner is preferred. Evidence of excellent interpersonal skills, effective communication skills, creative problem solving and excellent critical thinking and leadership; Previously demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary team; Effective change agent. .
Required Licenses: Registered Nurse License-NYS, Nurse Practitioner-NYS
Membership in pertinent professional organizations preferred
